Filing A Complaint

The procedures for filing a complaint are as follows:

File a complaint by post:
Download and print a complaint form, complete it, and mail it to
Office of the Ombudsman
Private Bag 13211
Windhoek, Namibia.

File a complaint telephonically:
Please call 061 207-3111 Mondays to Fridays from 08:00 to 17:00

File a complaint in person
Come to our office at the corner of Feld and Lossen Street in Windhoek and file a complaint in person. If you are from the South you can always visit our offices at Keetmanshoop. We also have regional offices in Ongwediva, Otjiwarongo and Swakopmund.

File a complaint by fax:
Download the complaint form, print and complete it, and fax it to 061 226-838.

The complaint form is available in  PDF  format. You can download the complaint forms below. Should you be dissatisfied with the outcome of your investigation or the conduct of any of our complaints investigators or the Ombudsman, you can also indicate this in a separate form.
